Frequently asked questions
Is patient data sent anywhere?
No. CFM Form Builder only processes blank form templates.
Patient fields in the generated .cfm are PS Suite keyword
placeholders — they're filled by PS Suite locally on your machine
after you import the file. Patient data never touches our servers.
What EMR does this work with?
Telus PS Suite only, for now. The generated .cfm files use
PS Suite's Custom Form format and import directly via the Custom Forms
menu.
Can I get a refund?
Credit-pack purchases are final. The 3 free credits you get when you sign up are there so you can confirm the Service works for your forms before you buy. See our purchase policy for billing-question contact details.
Do credits expire?
No. Credits you buy stay on your account until you use them.
What if I download the same form twice?
Re-downloads of the same PDF within 5 minutes are free — no credit decrement. After 5 minutes, each download counts as one credit. Edit your fields freely and re-download as needed within that window.
