What CFM Form Builder is

CFM Form Builder ("the Service") is a web tool that converts blank PDF clinical forms into Telus PS Suite Custom Form (.cfm) files. It is an independent tool and is not affiliated with Telus or PS Suite.

Intended use

• The Service is intended for healthcare providers who use Telus PS Suite EMR.
• The Service operates on blank form templates only. Do not upload PDFs that contain patient health information (PHI). Patient data is populated by PS Suite after you import the generated .cfm into your own EMR — patient data does not reach our servers.
• Do not upload PDFs you do not have the legal right to use.

What the Service is not

• Not a medical device. The Service does not provide clinical decision support and does not interpret medical content. It rewrites form layout only.
• Not an EMR. We never see patient data.

Credits and billing

The Service uses a credit model. New accounts receive three (3) free credits. Each new .cfm file you build and download consumes one credit. A re-download of the same blank PDF by the same account within 5 minutes does not consume an additional credit.

Additional credits are available in packs at the prices listed on the pricing page. Payments are processed by Stripe.

Your responsibility

You are responsible for reviewing every generated .cfm before using it with real patients. The detector and autopop assignments are best-effort heuristics; verify that the form behaves as intended in your EMR before relying on it.

Service availability

The Service is provided on an "as is" basis. We aim for high availability but make no uptime guarantee.

Account questions or termination

For account deletion, questions about these Terms, or any other matter, email support@cfmformbuilder.com. We aim to respond to billing questions within 1–2 business days and to product questions within 3–5 business days; we may take longer during clinic-busy stretches or extended absences.

Changes

The "last updated" date above reflects the most recent revision. Material changes will be communicated to active account holders.
